MY BIOGRAPHY
Dr. Hazelwood has served in the United States Airforce as an active duty orthopaedic surgeon in the Luke Air Force Base 56th Medical Group. Most recently, Dr. Hazelwood has supported the athletes of several collegiate and professional sports teams as a team physician.
